Drop off at 8am–allow 10 minutes for check-in. No food after 7pm the night before, water is ok until 7am.

Please arrive 10-15 minutes early for check-in and provide previous medical records.

 If urgent, please call us ASAP. Otherwise, please enter notes about illness and urgency.
